NOTICE TO ________________(NAME OF INS. CO.) PATIENTS

Effective January 1, 2002, Mason Medical Group will no longer participate with the following health plan: ________________ (NAME OF INS. CO.)

WHY DID OUR PRACTICE MAKE THIS DECISION?

Over the past several years, insurance companies have overstepped their bounds and in turn, have negatively impacted the rights of patients and physicians. They dictate facets of medicine such as the coverage of medical procedures, the lengths of hospital stays, the location of where x-rays can be taken, just to name a few. In other words, they are controlling the doctor-patient relationship. These actions of the health care industry are affecting the quality of care that we are committed to providing to you and your families.

We have tried to explain to plans such as _________ that we can no longer continue to compromise the quality of care that you have come to expect from us here in the office, especially in the name of profit for their own company. We are confident that you, as our patients, do not want our hands tied when it comes to providing you and your families with excellent care. We have made attempts at trying to reach a fair relationship with ____________, but with little success. You see it is our opinion that _________ is more interested in its own wallet than in your health care.

Therefore, it is with deep regret that after thoughtful and lengthy consideration we have found it necessary to end our relationship with ________ in the name of trying to protect our patients. We will continue to provide you and your families with the highest quality of care, however you may become responsible for more of the costs of this care after our termination is effective.

HOW LONG WILL THIS LAST?

We honesty don’t know. The Mason Medical Group is doing everything possible to resolve contractual issues with _________ so that this inconvenience is minimized.

WHAT CAN YOU DO TO HELP?

We encourage you to call your representative at _________ and voice your dissatisfaction with the plans interference with the doctor-patient relationship. Also talk to your employer and your state representatives about the deterioration of health care in the hands of managed care.

Above all, rest assured that our medical practice will work with you to ensure the quality and continuity of your care. Please feel free to ask our doctors or staff any questions you may have in regard to this very difficult decision. Also feel free to ask us about any other alternative health plans in which we continue to participate.
