Physician NPI and Form 855

Receipt of a National Provider Identification number is considered a "change of information" according to Medicare. As such, you will need to notify your Medicare carrier of this change by filing the CMS Form 855, which can be found at the CMS website - www.cms.hhs.gov/cmsforms.

The Form 855 must be filed within 90 days of receipt of your NPI.
