Common errors when billing a physician service

  1. Inputting incorrect information for the provider (name, address, etc)
  2. Inputting incorrect information about the patient (name, sex, date of birth, insurance info., etc)
  3. Using too little or many digits for ICD-10 codes
  4. Inputting incorrect information about the insurance provider (PO number, address, contact)
  5. Entering mismatched treatment and diagnosis codes
  6. Ignoring/forgetting to enter codes for services performed by the physician
